Summary:
This in not a remote position Student Care Navigator--This position provides mental health and physical health access support to all students of Lourdes.
This position will serve as the primary contact for any student who desires to access health care and will provider referral, access assistance, and follow up care.
This position will also lead campus-wide care teams focused on student health wellbeing.
ESSENTIAL DUTIES AND
Responsibilities:
Respond to all students requesting assistance accessing health care via text, phone call, email or office walk-in in a timely mannerProvide referrals to mental health and/or physical health providers in the communityAssist students to access healthcare services once referred; assist with setting appointments, arranging transportation, preparing for appointments, etc.
Follow up with each student who has initiated contactDocument all student contacts and outcomesDistribute and collect post-service satisfaction surveys and report findings as requestedLead Campus Care Team and Threat Assessment Team; participate as a member of the Covid Response TeamProvide staff, faculty, and/or student education related to health trends on campusADDITIONAL DUTIES AND

Prepare annual report for Ohio Department of Higher EducationParticipate in student activity events as requestedRequirements:
Minimum requirements:
Bachelor degree in nursing, counseling, social work, or a related field.
Licensed nurse, counselor, or social worker, preferred.
Strong communication skills and computer skills required.
